I, the undersigned parent/legal guardian of the above-named child, give permission for my child to sign themselves in and out of summer camp(s) and art or music classes at Monroe Street Arts Center (MSAC) and to leave the premises without being accompanied by an adult.
I understand and acknowledge the following:

Once my child signs out of camp/class and exits the MSAC facility, they are no longer under the supervision or responsibility of MSAC staff.

I accept full responsibility for my child’s safety, transportation, and well-being once they leave the MSAC premises.

I understand that MSAC staff will not monitor or confirm my child’s whereabouts after they sign out and leave camp/class.
Waiver of Liability:
By signing below, I agree to release, indemnify, and hold harmless Monroe Street Arts Center, its employees, volunteers, and representatives from any and all liability, claims, demands, or causes of action that may arise once my child has signed themselves out and left the facility. This permission remains in effect for the session(s) selected below, unless revoked in writing.
By checking here, you are consenting to the use of your electronic signature in lieu of an original signature on paper. You have the right to request that you sign a paper copy instead. By checking here, you are waiving that right. After consent, you may, upon written request to us, obtain a paper copy of an electronic record. No fee will be charged for such copy and no special hardware or software is required to view it. Your agreement to use an electronic signature with us for any documents will continue until such time as you notify us in writing that you no longer wish to use an electronic signature. There is no penalty for withdrawing your consent. You should always make sure that we have a current email address in order to contact you regarding any changes, if necessary.
